      Just kidding!  Required paperwork is the pits, and we know it.  So we've simplified it as much as possible.

You can download your Initial Evaluation Forms here (in both English and Español) -  Five pages total are included:  the Registration Form, the Medical Records Release, and the Past Medical/Surgical History Forms.

     If you have completed this paperwork prior to your first visit, please bring it with you, and plan to arrive 5-10 minutes early with your PT Script and your Insurance Card(s).

     If you were not able to complete this paperwork prior to your first visit, please plan to arrive 20-25 minutes early for 'Fun Paperwork Time' - and don't forget your PT Script and your Insurance Card(s).
